Clinical Journal One Essay The nursing process is an assessment, diagnosis, plan, implementation and evaluation of our patients. As a practical nursing student this is something we are learning and improving on daily. Assessment is the most important part of the nursing process. This process is becoming clearer every day spent in the clinical setting. The assessment part of the process is done with every interaction of our patients we are taught in school. I thought how could that be assessing something takes a considerable amount of time so assessing every patient, every interaction sounded absurd to me. Currently my clinical setting is a post surgical unit. The nurses on this unit are registered nurses with varying amounts of experience. As I observe the nurses, I have noticed they actually do assess everyone, every time. I was amazed how easy this skill seemed to them of course experience helps in that area. I had the opportunity to participate an assessment of a month old child. During this experience I was able to understand the doorway assessment I learned about in growth and development and it made more sense to me . I was able to take a set of vitals on this baby, weighed a diaper for the IO and was shown how to assess the IV site on an infant. I realize how very important a quiet baby is during the assessment because of this experience. Assessing an infant is different than adults. Infants are a bit harder to assess if they are not calm. Upset babies respirations will increase. I learned how to feel for the fontanel and dehydration signs in infants. There is so many parts to assessment and each and every part is an important step in keeping your patient on the path to recovery. Post-op assessment require that you check patients pedal pulse, this is something I would not have thought to be important but I have since discovered that pedal pulse is a warning sign for post-op patients. Theà smallest part of assessing a patient is just as important as the biggest parts. So a pedal pulse takes a minute or two to be sure itââ¬â¢s present and equal in both feet but the importance of doing this step in your assessment lasts forever because you deterred your patient a problem by doing a full head to toe assessment. Though, hands on assessments are most important, I have learned I do not always have to touch my patient to assess their condition. I can learn a lot from them just by listening to them and taking a good look at them. Patients also need more than a physical assessment. Physical head to toe is done at the beginning of your shift but you do and you can assess everyone with every interaction. There are many parts to assessment. The physical, of course, but as a nurse you must figure out the needs of each patient beyond the physical. They need emotional support as well. We must look at the whole picture when assessing. Do they need physical therapy? Home care? Support services? Are they going to need additional information on ways to be safe at home. Maybe the patients going to a rehabilitation facility after discharge so they may need to be assessed for which facility they wouldà gain the most from. We must assess if they require special equipment to improve on their independence. Assessing your patients doesnââ¬â¢t stop at taking a pulse or blood pressure.à So, assessment is very important and is easily done at every interaction once you see it done in the clinical setting. I still have so much to learn when doing a patient assessment but now I understand my role of assessing the patient as a licensed practical nurse and I understand the importance of every interaction assessment. Wolfgang Amadeus Mozart 1756-1791 Probably the greatest genius in Western musical history, Wolfgang Amadeus Mozart was born in Salzburg, Austria, Jan. 27, 1756, the son of Leopold Mozart and his wife, Anna Maria Pertl. Leopold was a successful composer and violinist and assistant concertmaster at the Salzburg court, whose archbishop, Sigismund von Schrattenbach, encouraged the activities of Leopold and his remarkable children. Wolfgang began composing minuets at the age of 5 and symphonies at 9. When he was 6, he and his older sister, Maria Anna, embarked on a series of concert tours to Europe's courts and major cities. Both children played the keyboard, but Wolfgang became a violin virtuoso as well. In 1762 the Mozart children played at court in Vienna; the Empress Maria Theresa and her husband, Emperor Francis I, received them cordially. Later the Mozart children displayed (1763-66) their talents to audiences in Germany, in Paris, at court in Versailles, and in London (where Wolfgang wrote his first symphonies and was befriended by Johann Christian Bach, whose musical influence on Wolfgang was profound). In Paris the young Mozart published his first works, four sonatas for clavier with accompanying violin (1764). In 1768 he composed his first opera, La Finta Semplice, for Vienna, but intrigues prevented its performance, and it was first presented a year later at Salzburg. In 1769-70, Leopold and Wolfgang undertook a tour through Italy. This first Italian trip culminated in a new opera, Mitridate, re di Ponto, composed for Milan. In two further Italian journeys he wrote two more operas for Milan, Ascanio in Alba (1771) and Lucio Silla (1772). In 1772, Archbishop von Schrattenbach died, to be succeeded by Hieronymus von Colloredo. The latter, at first sympathetic to the Mozarts, later became irritated by Wolfgang's prolonged absences and stubborn ways. In 1772, von Colloredo retained Wolfgang as concertmaster at a token salary. In this capacity Mozart composed a large number of sacred and secular works. Wishing to secure a better position outside Salzburg, he obtained permission to undertake another journey in 1777. With his mother he traveled to France, where he composed the Paris Symphony (1778); he could find no permanent position, however. His mother died in Paris. Regardless of who or what, everyone has felt sympathy or has been given sympathy. Euripides, a famous Grecian playwright wrote Greek tragedies known for eliciting sympathy from the audience. An example of this is Euripidesââ¬â¢ play Medea. Medea is one of Euripidesââ¬â¢ most well known characters Euripides created Medea to be a sympathetic character. Medea was originally published in Greek; therefore many aspects of the play may have been lost in translation as it is difficult to translate both the literal and implied meaning of the original play. Although many may dispute that Medea is a sympathetic character it is evident through, various sacrific ial acts of love, Medeaââ¬â¢s unstable mental state and role as a foreign woman in the ancient Greece society help Euripides portray Medea as a sympathetic character as conveyed by his social commentary. Sacrifices are often made in oneââ¬â¢s everyday life. However, it is the repercussions of these sacrifices that elicit sympathy from oneââ¬â¢s peers. Medea with, ââ¬Å"...her heart unhinged in her love for Jason...persuaded the daughters of Pelias to kill their fatherâ⬠(Euripides 8-10). Euripides uses a mythological allusion by briefly referring to the legendary journey of Jason and the Argonauts. Euripides portrays Medea as a character who selflessly victimizes herself. Through this sacrificial act of love Medea, victimizes herself by acquiring enemies on Jasonââ¬â¢s behalf. Furthermore, Medea betrays her... ...cal to further develop a clear understanding of her motives. Although Medeaââ¬â¢s actions were arbitrary and illogical, the reasoning behind her actions was completely selfless and motivated by love. Additionally, Medeaââ¬â¢s irrational state of mind contributes to her illogical selfless actions. Euripides demonstrates how Medea allows herself to be a victim to her anger which overpowers her rational thinking. Furthermore, Euripidesââ¬â¢ social commentary on the role of women and foreigners in the ancient Greece society display how Medea is poorly judged due to both these factors and her disinterest in conforming to societal norms. Therefore, it is apparent that Euripides created Medea as a sympathetic character. Although many aspects of the original play may have been lost in translation, it is proven that Medea is in fact a sympathetic character as shown through her plights.

Theories of Aging
For this part of the assignment I am going to be describing two theories of ageing. Firstly I will be describing the Disengagement theory and then the Activity theory. I will then be comparing the similarities and differences between the two, and also writing up two case studies of older people and explain the development that occurs in older life, relating back to the theories I will have discussed. Firstly the theory of Social Disengagement, disengagement means a personââ¬â¢s withdrawal from involvement with anything. The theory was first put forward by two authors Cumming and Henry in 1961 who believed that it was natural for the elderly to withdraw from social involvement with others, due to having restricted opportunities to interact with other people. There are many issues that limit social interaction which results in disengagement. Some of these may be things like retirement, ill health, mobility, travel or technology. The theory of disengagement was widely accepted as other theorists such as Bromley (1974) agreed with the theory arguing that ââ¬Å"although some individuals fight the process all the way, disengagement of some sort is bound to come, simply because old people have neither the physical not the mental resources they had when they were young. â⬠Secondly the Activity theory, this theory argues that older people need to stay mentally and socially active to limit the risks of disengagement. Being active in older life can help people to overcome many of the problems and issues they will have to endure throughout the older lifestage. Being active can include taking part in sports and activities, joining clubs and groups to go on trips, outings, holidays and even simple things like continuing with hobbies such as gardening or walking the dog. Being active is very important for many reasons when a person is in the later years of life. It is believed that itââ¬â¢s not enough to simple provide facilities for older people they must be educated to make use of them and encouraged to abandon fixed habits. The main argument for the activity theory is that disengagement can ultimately result in loss of physical and mental skills due to lack of practice. My first case study is of a man named Howard Lane, he died aged 75 years old and had been diagnosed with Alzheimerââ¬â¢s 9 years earlier. Over the 9 years his condition seriously deteriorated. Howard had had a very active life with various jobs such as a Clerk of Works at Par Docks, a mental nurse and other physically demanding roles. He had been married for 52 years and had two grown up children, a daughter Jennie who had two children and a son Richard who had four children. Howard was a very involved grandparent throughout the whole of his grandchildrenââ¬â¢s lives until his condition deteriorated so badly he couldnââ¬â¢t even tell who they were anymore. As Howard reached retirement he remained a very engaged active person despite the label he was now given as being ââ¬Ëold. ââ¬â¢ He very much fitted the role within the activity theory, regularly exercising, seeing his family, keeping in contact with them in a number of ways. He and his wife had a particular passion for ballroom dancingâ⬠¦ they had won competitions! He had a very healthy appetite, and had never smoked or drank at all throughout his life. Although Howard had always been healthy and had no previous health issues and had stayed active throughout his final lifestage he still developed the disease Alzheimerââ¬â¢s. As the illness progressed Howard gradually changed as a person. He became forgetful, got confused easily, and as he began to seriously deteriorate he became violent at times, physically incapable of doing things for himself, he would forget things that had happened and who people were. Further into his illness Howard became more and more disengaged. His whole life had changed due to the process of ageing along with the unfortunate illness he had. Not only had the disengagement affected Howard as a person it was also affecting a lot around him. Firstly the most obvious effect it was having was on Howardââ¬â¢s family. They began to be constantly worry about him therefore would be constantly in contact asking him if he was ok. It also put a large amount of stress on his wife Vera, and because she was in her older age as well she found it very hard to deal with the stress she was under. By the time Howard was the age of 71 Vera had no other option but to put Howard in a nursing home so he could receive the care that he needed. Due to Howardââ¬â¢s illness Vera had now become disengaged, she had spent all of her time looking after her husband, worrying about him and visiting him constantly. Another major factor that affected Veraââ¬â¢s stress was the fees she had to pay at the nursing home. She was very worried about the scale of the costs and was worried about having to sell her home. She had gradually lost contact with her friends and had no time for any personal hobbies or even time to properly look after herself and because of this Vera had become stressed and run down and was finding caring for Howard very hard to cope with. Her biggest worry and fear constantly being how she would cope with it if Howard died. Bereavement is usually hard to cope with for the elderly especially disengaged people, they are likely to feel more isolated and alone and this is what Vera was frightened of. In actual fact Vera died suddenly in December 2003, shortly followed by Howard June 2004. By this time Howard had no understanding of anyone around him as he had little response to anything, therefore he did not grieve for his wife as he didnââ¬â¢t even understand that she was gone. Their family described it as a blessing because in this way Vera never had to cope with the bereavement of losing her partner and neither did he. Overall retirement did have a positive effect on Vera and Howard to begin with. They had more freedom to be active, pursue their hobbies, spend time together, and with their loved ones. However old age brought ill health which caused Vera and Howard to disengage and become isolated from others which continued to bring ill health and stress. My second case study is about a woman named Ruth Cohen. Ruth Cohen is an 84 year old woman, she had previously been a teacher for nearly 40 years. Ruth has one son of 60 who has two daughters Lisa and Issie, Lisa has a son of 17. Ruth is a very old woman and has a number of health problems including arthritis, high blood pressure, and sight and hearing problems. However Ruth has lead a very healthy and active retirement. Since Ruth has retired she has done voluntary work for a number of charities and her local church as she is a very religious woman. Even in her late 70ââ¬â¢s Ruth attends church every Sunday and helped to run the local Sunday school. She regularly met her friends from church to go out for tea. She tries to visit her children and grandchild as much as she can however due to her eyesight she cannot drive anymore so her only way of seeing them is to get the bus. In her old age, Ruth despite still being so active has become quite frail and is frightened easily therefore tries to avoid going to places with people she doesnââ¬â¢t know, she tends to stay in her own village, go to the same shops and see the same people every day. Ruth was married for 60 years to James, unfortunately 14 years ago James passed away due to a heart attack. Ruth obviously had a hard time with the loss of her husband of 60 years however because of the lifestyle Ruth led she managed to carry on with her life and avoided becoming disengaged and easily accepted the support and help of ther whereas a lot of people cannot do so so easily. Because of Ruthââ¬â¢s personality and the way she was her family didnââ¬â¢t feel like they had to constantly worry about her and they knew she would be getting on fine without their constant care. However Ruth is getting rather old now so they do visit more frequently and her granddaughter Issie bought her a dog to keep her compan y and occupied. As Ruth entered her 80ââ¬â¢s the physical ageing process could no longer be avoided and began to take over her life. She became very weak and could only walk with a Zimmer frame, she became increasingly isolated in her home due to mobility issues and although people did visit she began to feel lonely and depressed.
